Case Management & Resource Coordination Services

We offer Intensive Case Management and Resource Coordination services for children (ages 5 and older). Our goal is to help children and their families successfully meet the challenges of daily living.

Our Case Management and Resource Coordination services educate families with children living with serious emotional and behavioral challenges on the supports available to them in the community. We focus on the child’s strengths to determine what services and supports will help them grow and learn to the best of their abilities. We offer assistance in the following areas:

• Physical and mental health
• Education
• Socialization

Available in Northampton and Lehigh Counties.
